Doctor Engine is an diesel who lives on on Sodor Island. He is a Doctor, hence the name "Doctor Engine."

- The fact that he has crash carts concealed beneath his roof is most likely a nod to an abandoned concept from the original scripts of Thomas and the Magic Railroad (which was preserved at one point of the book, Diesel 10 Means Trouble), in which Pinchy was concealed beneath Diesel 10's roof and not revealed until the scene where he is talking to Splatter and Dodge (especially considering the fact that he shares the same basis as Diesel 10).
